To sign up for a Group Health Plan, you first have to become a RHAND member.
Here’s how you can start:
- Complete GHP Enrollment Form
- Complete GHP Statement
- Complete Checkup Questionnaire (where applicable)
- 1 form of valid Identification.
- Proof of relationship for eligible family members (marriage certificate, birth certificate
- Common Law spouse: Common Law Certificate
- Group Health Statement: Dependents 15 & over | Dependents under 15
- Dependent child over 19: student letter required for child completing school; medical report for special needs child
- Dependent child 2 years & under: Pediatrician report
- Two months premium (amount based on coverage).
