To sign up for a Group Health Plan, you first have to become a RHAND member.
Here’s how you can start:
- Completed GHP Enrollment Form
- Completed Part 1 GHP Statement
- Applications for dependents and applicants aged 46 – 49 must complete Part 1 of the GHP Statement ONLY.
- Applicants aged 50 and over must complete Part 1 and 2 of the GHP Statement.
- 1 form of valid Identification.
- Proof of relationship for eligible family members (marriage certificate, birth certificate).
- Two months premium (amount based on coverage).
